About the Role
This position will provide care coordination to BHHP residents, during their stay and for 3-6 months following transition to next accommodation setting.

This role includes conducting comprehensive holistic health assessments and/or discipline specific assessments, coordinating health and social care needs and determining when specialist services are required, monitoring achievement of resident goals, facilitating intervention from other health care professionals and ensuring appropriate linkages with hospital, housing and community services. The residents will often present with complex psychosocial, aged and healthcare needs.

Classifications for this position will range from:

Registered Nurse, Grade 3B YU11 -YU12 ($99,299 per annum)

Community Health Nurse, Grade 4A YW7 -YW8 ($104,816 per annum)

Occupational Therapist, Grade 2 VF6 - VF9 ($83,361 - 95,825 per annum)

Physiotherapist, Grade 2 VB1 - VB4 ($83,361 - 95,825 per annum)

Social Worker, Grade 2 SC21 - SC24 ($83,361 - 95,825 per annum)


Successful 4A applicants will also:

  • Demonstrate significant care coordination experience working with residents with complex needs;
  • Demonstrate the capacity to act as ‘Shift Lead’ on weekends;
  • Will lead and participate in quality improvement activities;
  • Provide clinical supervision and support to more junior staff.

Your Contribution
Provide care coordination to a caseload of eligible residents, including operating on site and in an outreach capacity where required.

  • Within a client centred framework, undertake holistic comprehensive assessments in collaboration with the resident using motivational interviewing and self-management approaches.
  • Collaborative care planning with residents, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental, safety and economic needs.
  • To work collaboratively with all staff from the partnering organisations: Launch Housing and Brotherhood of St Lawrence, and other external agencies as required.

About Sumner House

The Sumner House site in Fitzroy has been used to support homeless people throughout the COVID-19 pandemic. All services at the site are delivered through a partnership between Brotherhood of St Laurence, Launch Housing and St Vincent’s, along with relevant government departments.The site reopened in response to Melbourne’s third wave of COVID in September 2021 to deliver the following functions:

  • The COVID Isolation and Recovery Facilities (CIRF), provides a safe and supportive environment for homeless people who are required to isolate due to being a primary close contact or due to being symptomatic for COVID-19.
  • The Better Health and Housing Program (BHHP) will support individuals with health issues who are experiencing chronic homelessness by providing an integrated health and homelessness service response for stays of usually 12-26 weeks. BHHP environment will support residents to rest and recover, and address health and social issues, according to the person’s own goals.
