The role:

We are looking for a highly organised professional with a commitment to exceptional, person-centred care to join the ECH team as a Care Coordinator.

As an ECH Care Coordinator you will build strong relationships with our Home Care Package clients and their families while you support them to navigate the aged care system. You will also build strong relationships with your colleagues across ECH as we work together to enable our clients to have the best life possible as they age.

What you’ll be doing:

ECH Care Coordinators:

  • Provide advice and guidance to enable informed decision making and self-determination.
  • Undertake comprehensive assessments of client needs and ensure goals are developed in line with those needs and their Home Care Package budgets.:
  • Identify when a client’s needs have changed and adjust services and support accordingly.
  • Understand ECH products and services and build them into clients Care Plans to meet our clients evolving needs.
  • Work in partnership with a multi-disciplinary team to provide the highest level of care to ECH clients.

You will bring to the team:

  • Commitment to helping older people retain their independence so they can remain at home in the community
  • Relevant experience and/or tertiary qualifications in health, social or behavioural sciences.
  • The ability to use initiative, think innovatively and continually look for ways to meet client needs.
  • Sensitivity to the needs of older people of different social, economic and cultural backgrounds.
  • A current driver’s licence
  • Experience and/or knowledge of Home Care Packages

It’d also be helpful if you had:

  • A current Police Check

The salary band for this position is between $69,877 & $75,092

ECH is a not-for-profit organisation in the Health Care Sector. This means that in addition to your salary you can also salary package up to $18,549 per annum. We also offer a range of perks through our ever-expanding Employee Benefits Program (eg. discounted gym memberships).

You will be provided with full training and support including all equipment that is required to succeed in this role, including a subsidised fully maintained vehicle.
