icare is currently seeking an individual with previous case management experience to support our dust disease participants in the community.

About the Role
As Case Officer you will report to the Case Management Team Leader and work collaboratively with internal and external stakeholders such as medical and service providers and participants. You will effectively and efficiently case manage client care needs for cases with relatively low levels of complexity, by assessing care or service requirements, develop, monitor and evaluate care plans and routinely adjust those plans as needs change.
